Thank you for choosing the Library's Memorial Book Program as a way to honor the memory of friends and family. Use this online form to initiate a memorial contribution. Please provide all the information requested below so we can follow your instructions. Please include the information exactly as you would like to appear on the bookplate.
 



It's simple to start your memorial today. You can use your checking account, debit, or credit card. Visa, MasterCard, Discover and American Express are accepted.  You do NOT need a PayPal account to use this service. You will receive an immediate receipt to print for your records as well as an email confirming the charge. For tax purposes, please note that your gift is fully tax deductible as provided by law.

If you prefer using the mail. Simply complete this form and submit.  Then print and mail the confirmation with your donation. The address is already on the confirmation form.
